Bring the following documents:

• Identification
• Social Security Card/ITINS for all household members
• Current Utility Bills of Electric, Gas, Water

(Present all three services )

• Proof of Gross Income for past 30 days for all household members
  • Pay stubs for current employment
  • IDES Unemployment Letter/UI Findings
  • Letter of DHS Benefits
• Additional documents can be requested at the time of application
